زمینه و هدف: سندرم نقص ایمنی اکتسابی از آلودگی بدن با ویروس نقص ایمنی انسانی ایجاد می‌شود. عوامل متعددی بر بقای افراد مبتلا به AIDS موثر هستند که اغلب با استفاده از مدل‌های رگرسیونی کاکس مورد بررسی قرار گرفته‌اند. پیش‌فرض استفاده از مدل کاکس، متناسب بودن مخاطرات است که در صورت تخطی، این مدل از کارایی لازم برخوردار نیست. هدف از این مطالعه، تعیین عوامل موثر بر بقای مبتلایان به AIDS  با استفاده از مدل کاکس توسعه‎یافته می‎باشد.
مواد و روش‎ها: در این مطالعه کوهورت گذشته‌نگر اطلاعات 1050 بیمار مبتلا به AIDS مراجعه‌کننده به مراکز مشاوره بیماری‌های رفتاری امام خمینی و زمزم تهران طی سال‌های 2004 تا 2014 بررسی شد و اثر عوامل بالینی و دموگرافیکی بر بقای بیماران با استفاده از مدل‌های کاکس تعمیم‌یافته مورد مطالعه قرار گرفت.
یافته‌ها: 79 درصد از بیماران را مردان تشکیل می‌دادند. میانگین سن مبتلایان 21/84±10/34 سال و میانگین زمان تشخیص بیماری HIV تا ابتلا به ایدز 53/64±15/23 ماه بود. 76 درصد از افراد سانسور شدند. متغیرهای سن بیمار در زمان تشخیص، راه­های انتقال بیماری، استفاده از ART و تعداد CD4 و زمان تشخیص بیماری تا ابتلا به ایدز در تحلیل تک‌متغیره و چندمتغیره معنی‌دار شدند (05/0>p).
نتیجه‌گیری: عوامل بالینی متعددی بر بقای بیماران مبتلا به ایدز تأثیرگذار است. مهم‌ترین این عوامل، استفاده از داروی ART و تشخیص زودهنگام بیماری است که شانس زنده ماندن مبتلایان را به‌طور چشمگیری افزایش می‌دهد. بنابراین به‌ نظر می‌رسد ارائه خدمات غربال‌گری و درمانی برای افراد پرخطر کاملاً ضرورت دارد.
