fa مقایسه اثر کلوبتازول و دوکسپین موضعی با کلوبتازول در درمان بیماران مبتلا به اگزمای دست عمومى General پژوهشي اصیل Original Atricle <p style="text-align: justify"><strong>  زمینه و هدف</strong>: اگزمای دست یک بیماری شایع است که اغلب نیاز به درمان طولانی مدت دارد . تاکنون چندین مطالعه در مورد اثربخشی دوکسپین موضعی در درمان درماتوزهای خارش دار صورت گرفته است.هدف از این مطالعه بررسی میزان اثربخشی پماد کلوبتازول 05/0 <u>درصد </u>و دوکسپین 5<u> درصد </u>با کلوبتازول به تنهایی در درمان اگزمای دست می باشد.</p> <p style="text-align: justify">  <strong>مواد و روش ها</strong>: در این مطالعه کارآزمایی بالینی 70 بیمار مبتلا به اگزمای متوسط تا شدید دست در طی سالهای 1390-91 انتخاب و به صورت تصادفی به دو گروه تحت درمان با کلوبتازول و کرم دوکسپین 5<u> درصد </u>و گروه کلوبتازول و دارونما تقسیم شدند. کیفیت درمان با استفاده از شاخص شدت اگزمای دست و شدت آنالوگ در ابتدای درمان و 8 روز بعد مورد ارزیابی قرار گرفت. آنالیز آماری به کمک نرم افزار SPSS 18.0 و آزمونهای تی مستقل و تی جفتی انجام شد.</p> <p style="text-align: justify direction: rtl"> <strong> یافته ها</strong>: درنهایت 56 بیمار مطالعه را کامل کردند که 32 نفر مرد و 24 نفر زن بودند. کاهش شاخص شدت اگزمای دست و شدت آنالوگ در دو گروه درمان و دارونما پس از 8 روز از لحاظ آماری معنادار بود  ولی کاهش شاخصهای فوق بین دو گروه درمانی تفاوت معنی داری نداشت.</p> <p style="text-align: justify">  <strong>نتیجه گیری</strong>: هرچند شواهدی وجود دارد کرم دوکسپین 5<u> درصد </u>در کاهش علایم و نشانه های درماتیت ها موثر است، مطالعات محدودی صورت گرفته است. ما توصیه می کنیم در یک کارآزمایی بالینی وسیع تر اثربخشی این دارو در درمان اگزمای دست مورد ارزیابی قرار گیرد.</p> <p style="text-align: justify"><span style="font-size: 14px"><span style="font-family: times new roman"><em> <strong> Background </strong></em>: Hand eczema (HE) is a common skin disease which often needs long term treatment.. There are several reports about the efficacy of topical doxepin in the treatment of pruritic dermal diseases. This study was undertaken to compare the efficacy and safety of 5% doxepin cream and clobetasol 0.05% versus clobetasol 0.05% alone in the treatment of hand eczema.</span></span></p> <p style="text-align: justify"><span style="font-size: 14px"><span style="font-family: times new roman"><em> </em><strong><em> Materials and Methods</em>: </strong>a randomized double blind placebo-controlled, study was carried out during years of 2011-2012.In total 70 patients with moderate to severe hand dermatitis were selected randomly to be treated with either the doxepin 5% cream+ clobetasol or clobetasol+placebo twice daily for 8 days. Efficacy assessments were performed at baseline and 8days after termination of therapy, according to hand eczema severity index (HECSI) and visual analogue scale (VAS). Statistical analysis was done using SPSS software, version 18.0 and statistical tests including paired T-test and independent T-test.</span></span></p> <p style="text-align: justify"><span style="font-size: 14px"><span style="font-family: times new roman"> <strong><em> Results</em>: </strong>Overall, 56 patients completed our study. Out of them 32 were male and 24 were female. The reduction of HECSI score and VAS was statistically significant in the treatment and placebo group (p&le0.05), but differences in HECSI and VAS reduction between treatment and control group were not significant.</span></span></p> <p style="text-align: justify"><span style="font-size: 14px"><span style="font-family: times new roman"><strong><em>Conclusion</em>: </strong>Although there is some evidence that doxepin 5% cream is effective in reducing the signs and symptoms of eczematous dermatitis, few trials have been performed.
