بررسی ارتباط عوامل زمینهای با تبعیت از درمان در زنان مبتلا به دیابت بارداری

<strong>زمینه و هدف:</strong> تبعیت بیماران دیابتی از رژیمهای توصیهشده درمانی یکی از مهمترین چالشها در کنترل دیابت است. تبعیت از رژیم غذایی نیز در زنان باردار مبتلا به دیابت بارداری پایین است. عدم پیروی میتواند سبب کاهش اثرات مفید درمانی، علائم و نشانهها، افزایش عوارض یا حتی مرگ شود. بنابراین انجام تحقیقات بیشتر در مورد عوامل تعیینکننده رفتارهای پیروی از رژیمهای درمانی ضروری به نظر میرسد. مطالعه حاضر با هدف تعیین ارتباط مشخصات جمعیتشناختی و بارداری با تبعیت از درمان در بیماران مبتلا به دیابت بارداری انجام شد.<br>
<strong>مواد و روش ها:</strong> این مطالعه توصیفی تحلیلی روی 260 نفر از زنان مبتلا به دیابت بارداری تحت درمان با رژیم غذایی مراجعهکننده به مراکز بهداشتیدرمانی و بیمارستانهای امامرضا(ع)، قائم و امالبنین(س) در شهر مشهد انجام شد که به روش نمونهگیری چندمرحلهای تلفیقی انتخاب شدند. دادهها با استفاده از پرسشنامه مشخصات جمعیتشناختی و بارداری و پرسشنامه پژوهشگرساخته تبعیت از رژیم درمانی گردآوری شده و سپس با آزمون رگرسیون خطی چندگانه و توسط نرمافزار SPSS نسخه 16 مورد تحلیل قرار گرفتند.<br>
<strong>ملاحظات اخلاقی:</strong> این مقاله حاصل بخشی از نتایج طرح تحقیقاتی مصوب در کمیته تحقیقات دانشجویی دانشگاه علومپزشکی مشهد با کد اخلاق IR.MUMS.NURSE.REC.1397.091 است.<br>
<strong>یافته ها:</strong> نمرات تبعیت از رژیم غذایی، پایش قند خون و تبعیت از ورزش اثر معنیدار و مثبتی بر نمره کل تبعیت از درمان داشتند (0/001>P). همچنین سطح تحصیلات افراد (0/044=P)، ناخواسته بودن (0/006=P) یا برنامهریزینشده بودن (0/021=P) حاملگی، اثر مثبت و معنیداری بر نمره کل تبعیت از درمان داشت. زنانی که سابقه دیابت در بستگان درجهیک (0/010=P) و نیز سابقه سقط (0/021=P) داشتند، از نمره کل تبعیت از درمان بالاتری برخوردار بودند. <br>
<strong>نتیجه گیری:</strong> برخی از مشخصات جمعیتشناختی و بارداری میتوانند در میزان پیروی از رژیم درمانی در زنان مبتلا به دیابت بارداری تأثیرگذار باشند و لازم است مراقبین سلامت به نقش این عوامل در بهبود تبعیت از رژیم درمانی بیماران توجه کنند.
<div style="text-align: justify;"><strong>Background and Aim:</strong> The adherence of diabetic patients to the recommended treatment regimens is among the major challenges in diabetes control. Dietary adherence is low in pregnant women with gestational diabetes. Non-compliance can reduce the beneficial effects of treatment, symptoms, and signs, increase complications, or even cause death. Therefore, further research on the determinants of follow-up behaviors is essential.<br>
<strong>Methods & Materials:</strong> This was a descriptive-analytical study. In total, 260 women with gestational diabetes treated with diet referring to healthcare centers, and Imam Reza (AS), Ghaem, and Umm Al-Banin Hospitals in Mashhad City, Iran participated in this research. The study subjects were selected using convenience, multistage, class, and cluster sampling methods. The required data were collected using a demographic and pregnancy profile questionnaire and a researcher-made questionnaire of adherence to the treatment regimen. The obtained data were analyzed by inferential statists and regression tests in SPSS v. 16.<br>
<strong>Ethical Considerations: </strong>This study was registered with ethics code of IR.MUMS.NURSE.REC.1397.091 in the ethics committee of Mashhad University of Medical Sciences.<br>
<strong>Results:</strong> The regression analysis results indicated that dietary adherence, blood glucose monitoring, and exercise adherence scores presented a significant and positive effect on the total treatment adherence score (P<0.001). Furthermore, education (P=0.044), unwanted (P=0.006), or unplanned pregnancy (P=0.21) provided a positive and significant effect on the overall score of treatment adherence. Women with a history of diabetes in first-degree relatives (P=0.010) and those with a history of miscarriage (P=0.021) obtained higher scores of total treatment adherence.<br>
<strong>Conclusion:</strong> According to the present study findings, some demographic and pregnancy characteristics can affect the rate of adherence to the treatment regimen in women with gestational diabetes. Healthcare providers need to pay attention to the impact of these factors in improving adherence to patients’ treatment regimens.<br>
