fa بررسی اثربخشی وکتورهای ویروس وابسته به آدنو (AAV) در ژن‌درمانی مدل‌های حیوانی اسکلروز جانبی آمیوتروفیک: یک مرور نظام‌مند Evaluating the Effectiveness of Adeno-Associated Virus (AAV) Vectors in Gene Therapy of Animal Models of Amyotrophic Lateral Sclerosis: A Systematic Review علوم پایه Basic Sciences مقاله مروری Review Article مقدمه: اسکلروز جانبی آمیوتروفیک (Amyotrophic lateral sclerosis) ALS یک بیماری تخریب سیستم ایمنی است که حاصل التهاب نورون‌ها می‌شود و در درجه اول به نورون‌های حرکتی آسیب می‌رساند. جهش در ژن SOD1 در ALS نقش دارد و بسیاری از پروتئین‌ها و عوامل را مختل می‌کند. ژن‌درمانی به عنوان درمانی برای ALS در مدل‌های حیوانی دنبال می‌شود و این مطالعه به دنبال بررسی نقش وکتور وابسته به آدنو (AAV) در این نوع درمان بود. روش کار: ما مطالعات را در یک دوره 5 ساله از 2020 تا آگوست 2025 در پایگاه‌های داده  PubMed، Scopus  و Web of Science بررسی کردیم. کلمات کلیدی خود را از پایگاه داده Mesh استخراج کردیم و مطالعه خود را بر اساس PRISMA نوشتیم. ما از ابزار SYRCLE’s برای ارزیابی خطر سوگیری استفاده کردیم. یافته‌ها: تعداد 21 مطالعه در این بررسی نظام‌مند گنجانده شد که همگی مطالعات حیوانی بودند و از خانواده AAV به عنوان ناقل استفاده می‌کردند. اکثر آنها سروتیپ 9 AAV بودند که به صورت داخل وریدی، داخل نخاعی و در بیشتر موارد به صورت داخل عروقی مغزی تزریق می‌شدند. این سویه‌ها توانایی حمل پلاسمیدهای کدکننده فاکتورهای مغزی و پروتئین‌های تنظیمی را نشان دادند که در نهایت باعث بهبود عملکرد حرکتی، تعادل اتصالات عصبی-عضلانی (NMJs) و در نهایت کاهش تجمع SOD1 جهش‌یافته در موش‌های مبتلا شد. نتیجه‌گیری: نتایج ما نشان داد که AAVها به عنوان ناقل، پتانسیل درمان ALS را دارند، این گروه توانایی عبور از سد خونی- مغزی را دارند، با این حال مطالعات فعلی محدود به جمعیت‌های حیوانی است. ما توصیه می‌کنیم که مطالعات بالینی مبتنی بر خانواده AAV به عنوان ژن درمانی برای بیماران ALS، یافته‌های ما را به چالش بکشد. Introduction: Amyotrophic lateral sclerosis (ALS) is a degenerative immune system disease that results from inflammation of neurons, primarily affecting motor neurons. Mutations in the SOD1 gene are implicated in ALS and disrupt many proteins and factors. Gene therapy is being pursued as a treatment for ALS in animal models, and this study sought to investigate the role of adeno-associated virus (AAV) in this type of therapy. Methods: We reviewed studies in 5 years from 2020 to 2025 in PubMed, Scopus, and Web of Science databases. We extracted our keywords from the MeSH database and wrote our study based on PRISMA. We used the SYRCLE’s risk of bias tool to assess the risk of bias. Results: Twenty-one studies were included in this systematic review, all of which were animal studies, and used the AAV family as vectors. The majority were AAV serotype 9, which was injected intravenously, intrathecally, and in most cases intracerebrovascularly. These strains showed the ability to carry plasmids encoding brain factors and regulatory proteins, which ultimately improved motor function, neuromuscular junctions (NMJs) balance, and ultimately reduced the accumulation of mutant SOD1 in affected mice. Conclusions: Our results showed that AAVs have the potential to treat ALS as vectors; this group has the ability to cross the blood-brain barrier; however, the current studies are limited to animal populations.
