مقدمه: جراحی قلب باز از جمله مداخلات پیچیده پزشکی است که با تغییرات فیزیولوژیک قابل توجهی همراه بوده و نیازمند مدیریت دقیق دارویی در دوره پس از عمل، به ویژه در بخش مراقبت‌های ویژه (Intensive care unit) ICU، می‌باشد. یکی از چالش‌های اصلی در این بیماران، دستیابی به اکستوباسیون به موقع و ثبات همودینامیک پس از توقف داروهای بیهوشی است. پروپوفول و دکسمدتومیدین از جمله داروهای رایج مورد استفاده برای آرام‌بخشی در این دوره هستند که هر یک دارای مزایا و معایب خاص خود می‌باشند. این مطالعه با هدف مقایسه اثرات پروپوفول و دکسمدتومیدین بر مدت زمان اکستوباسیون، آرام بخشی، بررسی مدت زمان بستری در بخش مراقبت‌های ویژه و مدت اقامت در بیمارستان در دو گروه دریافت‌کننده دارو بوده است.
روش کار: این بررسی یک کارآزمایی بالینی تصادفی است که بر روی 92 بیمار کاندید CABG که معیار ورود به مطالعه، انجام و بصورت تصادفی بلوکی به دو گروه دریافت کننده داروی دکسمدتومیدین و پروپوفول اختصاص داد شد. دو گروه از لحاظ سنی و جنسی همگن شده بودند. متغیرهای امتیاز رمزی، زمان اکستوباسیون بعد از ورود به بخش ICU، مدت بستری شدن در ICU، مدت زمان بستری در بیمارستان و مرگ یا ترخیص بیماران دو گروه ثبت شد. نتایج با استفاده از نرم‌افزار R و آزمون‌های اختلاف میانگین‌های دو گروه تحلیل گردید.
یافته‌ها: دو گروه از لحاظ سنی و جنسی همگن بودند. میانگین امتیاز رمزی در گروه دریافت‌کننده دکسمدتومیدین بصورت معناداری کمتر از این شاخص در گروه پروپوفول بوده است. میانگین زمان اکستوباسیون بعد از ورود به ICU در دریافت‌کنندگان دکسمدتومیدین بصورت معناداری کمتر از این شاخص در گروه پروپوفول بوده است. توزیع مرگ و میر در دو گروه دریافت‌کننده دارو همگن بوده است.
نتیجه‌گیری: مطالعه حاضر نشان داد که استفاده از دکسمدتومیدین در مقایسه با پروپوفول در بیماران جراحی قلب باز با مزایای قابل توجهی همراه بوده است، متوسط امتیاز رمزی کمتر، زمان اکستوباسیون در گروه دکسمدتومیدین به طور معناداری کوتاه‌تر بود که می‌تواند منجر به کاهش مدت اقامت در ICU و بهبود روند بهبودی بیمار شود. با توجه به یافته‌های فوق، به نظر می‌رسد دکسمدتومیدین می‌تواند گزینه مناسبی برای آرام‌بخشی پس از جراحی قلب باز باشد با توجه به تسهیل اکستوباسیون سریع‌تر باشد. با این حال، مطالعات بیشتر با حجم نمونه بزرگ‌تر برای تأیید این یافته‌ها و بررسی پیامدهای بلندمدت توصیه می‌شود.
